What is a hepatectomy?
Hepatectomy is surgical removal of part of the liver. ‘Major’ hepatectomy removes 3 or more Couinaud segments; ‘minor’ removes less.
How much liver can be safely removed?
In a healthy liver, up to 70% can be safely removed. In cirrhotic or diseased liver, only 30-40% may be safe. The future liver remnant must be adequate.
What is the future liver remnant (FLR)?
FLR is the volume of liver that will remain after resection. It must be at least 25-30% of standardised total liver volume (more in cirrhotic liver) to avoid post-operative liver failure.
What is portal vein embolisation?
If the FLR is inadequate, blocking the portal vein branch to the tumour side makes the other side grow over 4-6 weeks, allowing safer resection.
Can liver surgery be done laparoscopically?
Yes — laparoscopic and robotic liver resections are increasingly common, with proven benefits in blood loss, pain, hospital stay and cosmesis.
Does the liver grow back?
Yes — the liver regenerates to near-normal size within 6-12 weeks after resection, provided the remaining liver is healthy.
What are the risks of liver surgery?
Bleeding, bile leak, infection, liver failure and death. Risk is strongly correlated with surgeon experience and liver function.
How long is recovery?
Hospital stay is typically 5-8 days. Most patients return to normal activity in 4-6 weeks. Major resections may require longer.
Will I need blood transfusion?
Modern techniques (low CVP, vascular control, CUSA) allow most liver resections to be done with minimal or no blood transfusion.
What follow-up is needed after liver resection for cancer?
CT or MRI every 3-6 months for 2 years, then 6-monthly up to 5 years, along with tumour markers (AFP for HCC, CEA for colorectal metastases).
